When we began this book we thought a major Appendix would be a way of making good use of major things we have written from the past and enhancing our credibility.   As we have progressed with the book we think maybe this original idea is unnecessary and unwise.  The book may be more viable if not filled with so much past material.
Rather we could instead make these past materials available at a website associated with the book.  And this website might also offer a comments forum.   Here on this page we have kept the original Appendix idea and the list of things we had intended to include.
